Job Summary:

 

This is a Full Time, Direct Hire Position

The Case Manager is responsible for focusing on the four key areas of the “whole person:

1. Ensuring that the individuals we serve have a safe and secure place in which to live.

2. Helping them integrate into their community where they can develop healthy, productive relationships with friends and peers.

3. Assisting them in finding and obtaining employment or other productive interests.

4. Identifying and implementing supportive solutions to obstacles that prevent them from living meaningful and productive lives in our community, e.g. transportation. As a Service Coordinator you will be responsible for assisting and supporting between 30 and 40 individuals; depending on whether you support individuals who have chosen a service provider (vendor) to deliver services or have decided to self-direct their program of support. You will be responsible for ensuring that the needs of those individuals are being met and that we are doing so consistent with our internal policies and procedures and all applicable state and federal guidelines.




 

What We Need From You:

 

  • Bachelor’s degree or equivalent in Human Services or related field is preferred; Associates degree in Human Service, Psychology, Social Work  or related field is acceptable if accompanied by relevant experience.

  • Minimum of 2 years work experience in related position with responsibility for developing community, family and individual relationships, creating service plans, negotiating and budgeting resources, quality assurance, compliance, or other comparable areas.

  • Must demonstrate a high degree of compassion and understanding with a strong desire to help people

  • Demonstrated ability to establish and maintain trusting relationships with the individuals we serve, their families and other stakeholders. Serve as a trusted advisor and consultant to the individuals

  • you are assigned to serve. Guide the individuals you serve and their guardians through the process of obtaining the supports and services they will need to be successful.

  • Ability to assess needs and develop person-centered individual service plans. Ability to identify community resources, match individuals to those resources and monitor ongoing services for compliance with identified goals, applicable rules and regulations and desired quality outcomes.

  • Highly effective skills in critical thinking, problem solving and decision making

  • Ability to handle multiple tasks in a professional manner

  • Proficiency in office software applications; primarily the Microsoft Office suite of applications but other cloud enabled applications as well.

  • Ability to work independently but also collaboratively with other One Sky employees across departments and other stakeholders including direct service providers and individual guardians,

  • Previous experience working in the community with or as a member of other community based organizations is a strong plus but not a requirement

  • Strong priority management, planning and organizational abilities,

  • A demonstrated ability to be flexible and “think outside of the box” to to meet individual needs

  • Demonstrated skills in accounting, budgeting or financial services

  • Competency in logistics and cost estimating of services, ensuring that services are delivered in a timely, cost efficient way with appropriate outcomes

  • 20 days of ETO (combination of sick and vacation time) your first year.  Earned time starts accruing your first day at a rate of 6.7hrs per bi-weekly pay period.

  • Flexibility - This position can be done remotely as Service Coordinators are provided with a cell phone and company laptop.  In poor weather or when it is not feasible to get to the office, work can be done remotely.
