Personal Care Service Provider - Per Diem

Reporting to the Personal Home Services Supervisor, the Personal Care Service Provider (PCSP) provides personal care, other health-related services, homemaking services and companionship for client’s in their home as directed by the client’s care plan and under the direction of a Registered Nurse.

Essential Job Functions
Provides assistance to client with personal care such as bathing and grooming, dressing and undressing, meal preparation and feeding, transfers and ambulation, safe restroom use and continence and medication assistance. 
Completes homemaking tasks such as dusting, damp/dry mopping, vacuuming, cleaning, kitchen appliances, and cleaning bathroom fixtures. 
Assists the client with adaptive equipment or home exercise program. 
Socializes with clients with warm supportive (not personal) conversation, reading, games, cards, etc.
Communicates clearly and courteously with others including client, family members, and other agency staff.
Practices safe work habits and proper body mechanics.
Arrives on time to visits and completing visits within the scheduled time allowed.
Accurately documents care and services provided to the client using a mobile device according to Agency standards.
Attends Agency meetings and in-service programs.

Minimum Qualifications and Requirements

  • Graduate of a GED program or high school
  • Completion of the Agency's Personal Home Service Provider training program.
  • Reliable automobile 
  • Automobile Insurance
  • Valid Driver’s license
  • Ability to meet all Post-Offer Pre-employment requirements (including physical capacity exam, background checks, etc.)
  • If hired, be available Monday-Friday for the first weeks of employment to complete the required training.
  • Ability to understand and carry out directions and instructions
  • Ability to work with minimum supervision
  • Ability to read, write and speak English 


Work Environment
The employee will be exposed to a variety of uncontrolled work environments. The employee may be exposed to all kinds of weather conditions while driving to and from work assignments.  The employee may also be exposed to blood and bodily fluids, virus, disease and infection from patients and environmental hazards in the work environment.  The employee may be required to work extended periods of time using a mobile device.  


Physical Demands
This position requires the ability to frequently enter and exit and/or sit in an automobile for prolonged periods while commuting to work assignments.
Ability to talk and hear; read, write and speak in English, normal or correctable vision, and hearing.
Intermittent physical activity, including standing, walking, bending, kneeling, stooping and crouching as well as lifting and supporting patients.  
Ability to minimally lift up to 50 lbs. and the ability to provide up to 75% assistance for patient transfers.
Ability to frequently enter and exit and/or sit in an automobile for prolonged periods while commuting to work assignments.
