Community Partners

The In-Home Support (IHS) program provides assistance to families with clients ages 3 to 21 in the Self-Directed Program. Our supportive team is looking for candidates who are self-starters, have flexible hours, and are excited to make a positive difference in others’ lives. We are seeking to fill the following direct care positions:

 

Personal Care Provider (Req #425) to work with a 16-year-old young lady in Barrington, NH. Provider would work with the individual on personal care goals such as activities of daily living and communication. This client has a developmental disability and is nonverbal. Experience in working with individuals with disabilities is preferred. Provider would need to be able to assist with physical transfers and help with toileting. This Saturday position (specific hours to be discussed during interview) offers a salary starting at $12/hour.

 

Personal Care Provider (Req #428) to work with an 11-year-old male with a diagnosis of Autism. Days and times may vary and will be discussed directly with the family. Family is looking for a provider who is active and willing to bring client out into the community and to his extracurricular activities. Client would feel more comfortable with a male provider. Experience in working with individuals with disabilities is preferred. This 10- to 12-hours/week position offers a salary of $10/hour.

 

Personal Care Provider (Req #434) to work with a 13-year-old male and a 15-year-old female (siblings) who live in Dover, NH. Provider would work with them on personal care goals such as activities of daily living and communication skills building. Both clients have a developmental disability. Experience in working with individuals with disabilities is preferred. Services will be provided at the clients’ home. This 10-hours/week (weekends; hours to be discussed during interview) offers a salary of $15/hour.

 

Personal Care Provider (#435) to work with a 13-year-old female who lives in Somersworth, NH. Provider would work with her on personal care goals such as activities of daily living and communication skills building. This client has a developmental disability. Experience in working with individuals with disabilities is preferred. Provider would need to be able to assist with physical transfers and help with toileting. Services will be provided at the client’s home. This 8.5-hours/week (specific days and hours to be discussed during interview) offers a salary of $12/hour.

 

Personal Care Provider (#455) to work with an 11-year-old male with a diagnosis of Autism. Provider would greet client as he returns home on the school van, communicate with the driver and monitor as needed, facilitate afternoon leisure activities while working on activities of daily living skills goals, make dinner, and supervise showering. Client is an extremely easy going, sweet, loveable, and active 6th grader who attends private school for children and adolescents with ASD. Provider must be someone who is enjoys playing backyard sports, and going to the park or other adventures during the afternoons! Experience, exposure, or interest in working with individuals with developmental disabilities is preferred. This 15-hours/week (Monday-Friday; 4:00 PM – 7:00 PM) offers a salary of $15/hour. Additional weekend hours are available.

 

Respite Provider (Req #471) to work with a 31-year-old woman who lives in Barrington, NH. As her respite provider, you will with her on Saturdays and/or Sundays (times are flexible) doing recreational activities to allow for her mother to take some time rest. The client enjoys going to Paint 4 Fun, Chuck E Cheese, the movies, the mall, etc. Prior experience in working with individuals with developmental disabilities is preferred but is not required. This 5-hours/week position offers a salary of $15/hour.

 

Personal Care Provider (Req #485) to work with a 17-year-old man with intellectual disability. As his provider, you will work with him on goals set forth as part of his In-Home Support plan. Goals will focus on community participation including practicing taking the COAST bus to certain activities and completing in-home PT exercises which have been established by his physical therapist. The family is looking for 3 to 4 days a week of support during after school/evening hours. Specific days and times are flexible and will be discussed with the family. Prior experience in working with individuals with intellectual disabilities is preferred but is not required. This 11.5-hours/week position offers a salary of $14/hour.

 

Personal Care Provider (Req #487) to work with two clients (siblings) who are 13 and 15 years of age. As their provider, you will work with them on personal care goals such as activities of daily living, behaviors, and communications. Both clients have a diagnosis of Autism. Prior experience in working with individuals with disabilities is preferred but is not required. Hours will take place on occasional weekends/evenings in Dover, NH. This 10-hours/week position offers a salary of $15/hour.

 

Personal Care Provider (Req #492) to work with an energetic 7-year-old boy who lives in Rochester. As his provider, you will work with him on various goals around activities of daily living and safety awareness. The client’s primary diagnosis is Down Syndrome and ADHA, though he does experience other special health care needs. He has a feeding tube that the provider would need to be willing to learn how to access and utilize. The hours for this position will be 3:15PM to 5:30PM, Monday-Friday, with additional hours later in the evenings or on the weekends. A Behavior Technician will also be in the home in the afternoon for after-school ABA therapy. This 10- to 15-hours/week position offers a salary of $13/hour.

 

Personal Care Provider (Req #494) to work with a 15-year-old male who lives in Farmington and ha Autism. As his provider, you will work with him on goals centered around activities of daily living such as: safety awareness, snack/lunch preparation, hygiene, and social skills. These activities will take place primarily in his home but may potentially include the community as well. Experience in working with individuals with Autism is preferred but not required. Provider must be comfortable with dogs as the family has two. While the position may become open prior to 01/01/2017, specific days would be: Wednesday to Friday from 2:30 PM to 6:00 PM plus school vacations. This 12-hours/week position offers a starting salary of $10/hour.

 

A High School diploma or equivalent is required for all of these positions.

 

As local travel is a requirement for these positions, candidate must have a valid driver’s license, clean driving record, proof of adequate vehicle liability coverage, and reliable transportation.

 

All candidates must pass a background check which includes criminal and driving records.

In the Self-Directed Services program, families in Strafford County manage their own services (including staffing) for an adult family member who has a developmental disability or a traumatic brain injury. The employee will work out of the family home and the family is the Managing Supervisor with Community Partners as the Employer of Record.
