Clinical Case Manager (Concord)

NFI North is seeking a Clinical Case Manager for the Transitional Housing Program in Concord, NH. This program is a day treatment and residential program providing care to adults with a prolonged history of mental illness and hospitalization.
Responsibilities: provide case management and clinical services to assigned participants, administrative duties and clinical support for participants, including supported employment as well as providing clinical training and support for staff. Responsible for all financial information and benefits management.
Bachelors Degree in Human Service field with four years related experience and/or training. Experience with supportive employment activities and the client population preferred.
