Participant Directed and Managed Services (PDMS) provides assistance to clients age 21 and older who participate in our Self-Directed Services program. Job & Life Skills Coach (Req #414#415) for a 32-year-old man with a traumatic brain injury who lives in Strafford County. Some of this man’s interests include swimming, taking walks, engaging in recreational activities, and looking for opportunities to volunteer in the community. The family is looking for a motivated, dependable, and responsible individual who will support this man with engaging in his daily program activities, as well as accessing his community and safely encouraging increased independence. This position will involve providing support and assistance with activities such as, but not limited to: accessing the community, working on physical fitness and mobility, assisting with personal care and ADL’s, seeking and maintaining volunteer employment opportunities, developing and maintaining and building relationships. The ideal candidate will have experience working with individuals with developmental disabilities or in education, and should be comfortable identifying and meeting support needs and problem solving. Additionally, the candidate should be someone who is willing and eager to be an advocate for this man. A good sense of humor and good communication skills are required. There are both 20- and 40-hours/week positions available (hours and days to be discussed upon interview).

 

Job & Life Skills Coach (Req #419) for a young woman in Durham, NH. The position would include working with the young woman on tasks such as: learning local bus routes, as well as exploring bus and train travel to and from Boston/Marblehead MA. Additionally, this position would include assisting her to research getting a driver’s permit. The ideal candidate would have experience working with individuals with disabilities and is comfortable identifying support needs, meeting those needs, and also initiating new ways in which the client can integrate herself successfully in her community. The family is looking for someone who is energetic, observant, a good communicator, and able to make great community connections on behalf of the client. This 3-hours/week position offers a salary of $15/hour.

 

Job & Life Skills Coach (Req #436) to provide weekly community-based support to a young man in Rollinsford, NH. The position would include working with the young man on tasks such as: going to the gym and support him at different therapeutic activities, supporting at hypnotherapy activities, and participating in other recreational and social activities in the community. The ideal candidate must have experience working with individuals with disabilities & behavioral health challenges. They will need to feel comfortable establishing clear boundaries with the client, comfortable identifying support needs, meeting those needs and also initiating new ways in which the young man can integrate himself successfully in his community. The ideal candidate will be someone who is energetic, observant, a good communicator and able to make great community connections on behalf of the man they support. This 30-hours/week position offers a flexible schedule including some nights and weekends and a salary starting at $13/hour.

 

Job & Life Skills Coach (Req #453) to provide support to a 29-year-old female with an intellectual disability who lives in Strafford County and receives support through the Agency’s Self-Directed Services program. Some of this woman’s interests include music, going to the gym, volunteering, and going to the movies. The ideal candidate is a motivated, dependable, and responsible individual who will this woman with engaging in her daily program activities, as well as accessing her community and safely encouraging increased independence. This position will involve providing support and assistance with activities such as, but not limited to: accessing the community, working on physical fitness and mobility, assisting with personal care and ADL’s, seeking and maintaining volunteer employment opportunities, developing and maintaining and building relationships. This 20-hours/week position (hours and days to be discussed upon interview) offers a starting salary of $12/hour.

 

Job & Life Skills Coach (Req #467) to provide weekly community-based support to a man in Barrington, NH. This position will involve providing support and assistance with activities such as: accessing the community and the gym, maintaining current employment, therapeutic horseback riding, and increasing social, communication, and independent living skills. The ideal candidate will be someone who has experience working with individuals with disabilities and who is energetic, observant, and a good communicator. This 35-hours/week position (hours/days to be discussed upon interview) offers salary of $12/hour.

 

Job & Life Skills Coach (Req #468) to provide weekly community-based support to a man in Barrington, NH. This position will involve providing support and assistance with activities such as: accessing the community and the gym, maintaining current employment, increasing social and communication skills, driving practice, and cooking/meal planning. The client’s interests include: music, reading, history, science, and arts & crafts. The ideal candidate will be someone who has experience working with individuals with disabilities and who is energetic, observant, and a good communicator. This 7- to 10-hours/week positions (hours/days to be discussed upon interview) offers a starting salary of $13/hour.

 

Job & Life Skills Coach (Req #473) to provide support for a young women and assist her with participating in community activities in a meaningful way, developing social skills, encouraging self-advocacy in positive manner as well as helping with practicing of coping strategies, as needed. Responsibilities include driving to and participating in exercise classes, community activities, and assisting with volunteer opportunities, social skills activities, and life skills workshops. The family is seeking a motivated, dependable, and responsible individual with an understanding of mental health. This is a 15-hours/week position (hours/days to be discussed upon interview) with a starting salary of $15/hour.

 

Job & Life Skills Coach (Req #495) to provide support for a 32-year-old male with a traumatic brain injury, who lives in Strafford County and receives support through the Agency’s Self-Directed Services program. Some of this man’s interests include swimming, taking walks, engaging in recreational activities, and looking for opportunities to volunteer in the community. The family is seeking a motivated, dependable, and responsible individual to provide supports in both his home and the community; with a focus around assisting to engage in his daily program activities, as well as accessing his community and safely encouraging increased independence. This position will involve providing support and assistance with activities such as, but not limited to: accessing the community, working on fitness and mobility (including swimming), seeking and maintaining volunteer employment opportunities, support with accessing social and recreational activities, support and coaching to effectively communicate, and work through communication difficulties. Additionally, this individual will also provide support with personal care; ADL’s and care of assistance dog. The family is looking for 9 hours of support per week; Monday’s 7:30 AM to 4:30 PM. Very competitive wage is provided.

 

All positions above offer reimbursement for mileage.

 

A High School diploma or equivalent is required for all of these positions.

 

As local travel is a requirement for these positions, candidate must have a valid driver’s license, clean driving record, proof of adequate vehicle liability coverage, and reliable transportation.

 

All candidates must pass a background check which includes criminal and driving records.

In the Self-Directed Services program, families in Strafford County manage their own services (including staffing) for an adult family member who has a developmental disability or a traumatic brain injury. The employee will work out of the family home and the family is the Managing Supervisor with Community Partners as the Employer of Record.
