The heart of our non-profit Agency’s mission is empowering the people we serve to live a full life in their community. Participant Directed and Managed Services (PDMS) provides assistance to clients age 21 and older who participate in our Self-Directed Services program. Our supportive team is looking for candidates who are self-starters, have flexible hours, and are excited to make a positive difference in others’ lives.

Personal Care Provider (Req #335) to provide mobility assistance to a client while he participates in recreational activities such as going to the gym and attends appointments with doctors and therapists. The client has an assistance dog who goes everywhere with him. The family is looking for a candidate who has patience, a great sense of humor, and enjoys coaching and encouraging an active lifestyle. This is a 2-days/week (Saturday and Sunday), 20-hours/week position.

Job & Life Skills Coach (Req #410) for a 21-year-old female who lives in Strafford County. Some of this young woman’s interests include crafts, fashion and animals. As this client’s coach, you will assist this young woman with exploring a variety of community-based activities, which will include going to the gym, taking part in therapeutic horseback riding, and exploring volunteer opportunities. This position will also include providing supports around personal care and daily living skills. This position does require experience working with individuals with Autism, and the ideal candidate should be comfortable identifying and meeting support needs and problem solving. Additionally, the candidate should be someone who is willing and eager to be an advocate for this young woman, feels comfortable modeling appropriate behaviors, and is comfortable working independently. An upbeat attitude and good communication skills are required. The family is looking for 30 hours of support per week; Monday – Friday, exact hours to be discussed upon interview. This is a benefits eligible position, and includes mileage reimbursement.

Job & Life Skills Coach (Req #414) for a 32-year-old male with a traumatic brain injury, who lives in Strafford County and receives support through the Agency’s Self-Directed Services program. Some of this man’s interests include swimming, taking walks, engaging in recreational activities, and looking for opportunities to volunteer in the community. The family is seeking a motivated, dependable, and responsible individual to support this man with engaging in his daily program activities, as well as accessing his community and safely encouraging increased independence. This position will involve providing support and assistance with activities such as, but not limited to: accessing the community, working on physical fitness and mobility, assisting with personal care and ADL’s, seeking and maintaining volunteer employment opportunities, developing and maintaining and building relationships. The family is looking for 20 & 40 hours of support per week; hours and days to be discussed upon interview. The ideal candidate will have experience working with individuals with developmental disabilities or in education, and should be comfortable identifying and meeting support needs and problem solving. Additionally, the candidate should be someone who is willing and eager to be an advocate for this young man. A good sense of humor and good communication skills are required.

Job & Life Skills Coach (Req #419) to provide weekly community-based support for a young woman in Durham, NH. The position would include working with the young woman on tasks such as: learning local bus routes, as well as exploring bus and train travel to and from Boston/Marblehead MA. Additionally, this position would include assisting individual to research getting a drivers permit. The ideal candidate would have experience working with individuals with disabilities and is comfortable identifying support needs, meeting those needs, and also initiating new ways in which the client can integrate herself successfully in her community. The family is looking for someone who is energetic, observant, a good communicator, and able to make great community connections on behalf of the client. Transportation will be a requirement of this position, so a valid driver’s license, clean driving record and driver’s insurance is required.

Job & Life Skills Coach (Req #436) to provide weekly community-based support to a young man in Rollinsford, NH. The position would include working with the young man on tasks such as: going to the gym and support him at different therapeutic activities, supporting at hypnotherapy activities, and participating in other recreational and social activities in the community. This position is for 30 hours/week with a flexible schedule including some nights and weekends. The ideal candidate must have experience working with individuals with disabilities & behavioral health challenges. They will need to feel comfortable establishing clear boundaries with the client, comfortable identifying support needs, meeting those needs and also initiating new ways in which the young man can integrate himself successfully in his community. The ideal candidate will be someone who is energetic, observant, a good communicator and able to make great community connections on behalf of the man they support. Transportation will be a requirement of this position, so a valid driver’s license, clean driving record and driver’s insurance is required.

Job & Life Skills Coach (Req #444) to provide support and assistance to a client with activities such as, but not limited to: accessing the community, accessing the gym/aquatic therapy, maintaining current employment, assisting with personal care and ADL’s, seeking and maintaining volunteer and paid employment opportunities, developing and maintaining and building relationships. The family is looking for 18 hours of support per week; hours and days to be discussed upon interview. The ideal candidate will be an individual who has experience working with individuals with disabilities and is comfortable identifying support needs, meeting those needs and also initiating new ways in which the man can integrate himself successfully in his community. The ideal candidate will be someone who is energetic, observant, a good communicator, and able to make community connections on behalf of the man being supported. Transportation will be a requirement of this position, so a valid driver’s license, clean driving record and driver’s insurance is required. Mileage reimbursement is provided.

Job & Life Skills Coach (Req #449) to provide support and assistance to a young woman with exploring a variety of community-based activities. These activities include swimming at the gym, working with a personal trainer for physical therapy, ASL training (if not already attained), taking part in a community-based craft group, providing support while at volunteer position at stables (to include some riding), volunteering at the food pantry, working with a job counselor to work towards job placement, and going to the public library. The ideal candidate will have experience working with individuals with developmental disabilities, and/or will have a basic understanding of ASL. However, the family is willing to train the right person. Candidate must be comfortable identifying and meeting support needs, problem solving, and working independently. Additionally, candidate should be someone with an excellent work ethic, who is willing and eager to be an advocate for this young woman, and feels comfortable being a role model and demonstrating how to put the best self forward. An upbeat attitude and good communication skills are required. The family is looking for 35 hours of support per week (1 hour on Monday for planning, and 8:30 AM - 5:00 PM Tuesday – Friday). This is a benefits eligible position, and includes mileage reimbursement.

A High School diploma or equivalent is required for all of these positions.

As local travel is a requirement for these positions, candidate must have a valid driver’s license, clean driving record, proof of adequate vehicle liability coverage, and reliable transportation.

All candidates must pass a background check which includes criminal and driving records.

In the Self-Directed Services program, families in Strafford County manage their own services (including staffing) for an adult family member who has a developmental disability or a traumatic brain injury. The employee will work out of the family home and the family is the Managing Supervisor with Community Partners as the Employer of Record.
