Provide nursing functions to support the health, wellness, and disease management of Health First clients.
Responsible for nursing care at patient appointments, such as obtaining vital signs, patient history, patient teaching, administering of oral and injectable medication and treatments within the scope of orders from providers. Perform venipuncture blood draws and urine collection as well as other diagnostics for lab work as requested by the provider.
Maintain client data and health information in the client's electronic medical record and generate patient related reports. Manage sample, stock, and DAP medications as well as immunization supplies. Participate in performance improvement and disease management programs. Assist in the coordination of well child and immunization programs. Assists in ordering and stocking of clinical supplies. Acts as back-up vaccine program manager.

Demonstrate competencird in nursing clinical functions. Ability to demonstrate knowledge and skills necessary to provide care/services to patients throughout the life span ages. Ability to read charts, monitors, and patient medical record data.
Computer skills and documentation in electronic medical records

| Education Requirements |
Compliance with regulatory requirements and standards of nursing practice, health and safety requirements, as well as professional educational requirements.
Successful completion of a Nursing Education Program and Nursing License for the State of NH.
