Assist providers and confer with patients in regards to nutritional plans. Provide scheduled nutritional therapy sessions for patients. Assist clinical staff with disease management programs such as the nutritional support for the diabetes program and the pre-natal program. Accesses and refers client's to the appropriate community nutritional supports such as food stamps and WIC.
Nutritional liaison for the CHAN network. Assist with community nutrition awareness such as presentations and support groups for Diabetes clients.

| Job Requirements |
Needs to demonstrate competency in functions of a registered dietitian. Demonstrates ability to assess nutritional needs of patients, and ability to demonstrate knowledge and skills necessary to provide care/services to patients throughout the life span ages. Demonstrates compliance with regulatory requirements and standards of practice, health and safety requirements, as well as professional educational requirements.
